Approve the draft you actually reviewed

Approval of an earlier draft does not automatically approve a replacement file.

A practical review sequence

1. Check the filename and version before deciding.
2. Resolve outstanding comments or explicitly accept them.
3. Upload a replacement as a new version and request its own decision.
